  • UnitedHealthcare Choice: This plan covers in-network care and allows policyholders to see in-network specialists without a referral. Out-of-network services are not covered.
  • UnitedHealthcare Choice Plus: This option covers in-network care, allows policyholders to see in-network specialists without a referral and provides some out-of-network coverage.
  • UnitedHealthcare Options PPO: This plan provides in-network and out-of-network coverage and allows policyholders to see a specialist without a referral. Out-of-pocket costs are lower for in-network care.

Cost of Addiction Treatment Under UnitedHealthcare/Optum

The cost of addiction treatment under UnitedHealthcare/ Optum depends on:

  • Your insurance plan
  • The type of treatment
  • Where you went to treatment

Many insurance plans have a deductible or a set amount a policyholder must pay before coverage kicks in. Once a deductible is met, patients may be responsible for paying a copayment, a coinsurance or both.

Copayments are fixed dollar amounts owed for healthcare services. For example, a patient may have to pay a copay of $25 for a physician’s office visit. Coinsurances are the percentage of treatment costs a patient is responsible for. That means a patient with a 30% coinsurance is responsible for paying 30% of their treatment costs. The insurance provider pays the remaining 70%.

Most insurance plans also have an out-of-pocket maximum. This is the maximum dollar amount a policyholder can pay toward treatment costs. However, when the out-of-pocket maximum is met, the insurance policy must cover all healthcare service costs for the rest of the year.

You can find out what your UHC policy covers by calling the number on the back of your insurance card. If applicable, a representative will tell you your:

  • Deductible
  • Copayment
  • Coinsurance amounts
